DONATION REQUEST FORM

To request funds or donations from Eagle Club, it is required that you fill out the Donation Request Form by clicking below. 

Requests may take up to 30 days to approve/deny.  Approvals will be limited by Eagle Club's annual budget and considered on the basis of the number of students/staff affected, whether or not prior fundraising has been attempted for this item, whether the group requesting has had prior donations from Eagle Club, and whether the group requesting has worked concession shifts this school year.  

NOT ALL REQUESTS WILL BE APPROVED.
